Q: What do I need to bring with me to my doctor’s appointment?

There are a few things that you can bring to each of your appointments that will help make sure the visit goes smoothly and all information collected is accurate.

  • Your insurance card and a photo ID. This is especially important for your first visit and your first visit of a new year. This will help ensure that all insurance information is up to date so your insurance claim is filled timely with accurate information.
  • Your medications. A medication list is helpful but the actual medication bottles are even better. This helps us to know exactly what medication, strength and dose you are taking. Over the counter medications, vitamins and supplement bottles are important to bring too.
  • Medical History Update. If you have seen a specialist or another physician and have had testing or medication changes this will be important for us to know so your record can be updated. If it’s your first visit at the clinic we will make arrangements with you to obtain your medical records from your previous physician prior to your first appointment.
  • A list of questions. It’s always good to bring a list of questions listed in order of importance to make sure all of your problems and questions are addressed.
