Our Long-Term Care Household Model

Our Household Model is nationally recognized. Long-term care residents live in small households of 12-24 people who share meals, activities, and living spaces together. Each household has a small team of dedicated staff — lead by a household coordinator and a clinical coordinator — so the residents and staff grow to know one another.

Each household is set up like an independent home, with a front door and doorbell, living room, dining room, baths, and bedrooms. Meadowlark has six households, each with a unique personality and layout.
