Kristen Martin, Care Transitions Leader

Kristen Martin, a dedicated member of the Meadowlark team, serves as a Care Transitions professional, seamlessly integrating her skills to facilitate smooth transitions for residents across the Meadowlark Hills community. In her role, Kristen takes charge of completing sales processes, managing referrals for transitional care and 24-hour skilled nursing, and collaborating with house teams to streamline the move-in process for residents and guests. As an integral part of the sales team, Kristen plays a crucial role in assisting individuals and their families in making informed decisions regarding their living experience and connection with Meadowlark Hills.

Kristen's journey with Meadowlark began in 2011 when she joined as a Household Coordinator in Special Care. Progressing through various roles, she assumed the position of Social Services Leader, showcasing her commitment to resident care. In October 2015, Kristen embraced the Care Transitions Leader position, leveraging her diverse experiences to contribute to the community's growth and development.

Educationally, Kristen holds an associates of science degree from Cloud County Community College in 2009 and earned her bachelor's degree in social work from Kansas State University in 2011. Kristen's commitment to education extends to her role as an Internship Instructor for Social Work students at Kansas State University.

Living in Wakefield, Kan., with her husband, Josh, Kristen finds joy in family life with their two children, Easton and Ellie, and three dogs, Bella, Keilah, and Odie. Kristen's love for Kansas State University sports, particularly football, is deeply rooted in family tradition, creating cherished memories that she now continues with her own family.

For Kristen, the hallmark of working at Meadowlark Hills is the friendly atmosphere among residents and staff. She considers her job as her 'home away from home with her second family,' expressing gratitude for being part of an organization that values the meaningful relationships between staff and residents. Kristen's dedication to creating a warm and supportive environment aligns seamlessly with Meadowlark's commitment to fostering a sense of community and well-being.
