Covering everything from general care plans to Parkinson’s disease, Meadowlark’s specialized health programs give you individualized care for your specific health and wellness needs. Most programs are private pay, and some are even available for non-residents.

Care Management
An invaluable resource to Meadowlark residents and local citizens, the Care Management program is a complete care service that helps you take control of your health and wellness decisions, while putting your family at ease. With a focus on creating a personalized care plan, this service is great for anyone, at any stage of life.
While the program can include a variety of services, four areas are typically provided:
- Conducting an in-person assessment
- Making a care plan
- Arranging services
- Monitoring needs
By assessing your daily activities, medical condition, mental status, safety, and financial resources, our staff, including our on-campus physician and registered nurse, can create and implement a long-term care plan to best serve your overall health.

Helpful Living
Whether you are in independent or assisted living, or are at home in the Manhattan community, Helpful Living is a caregiver service that provides just enough support so you can remain independent and thrive at home. Managed through Meadowlark’s Home Health Services (link to Home Health Services), Helpful Living caregivers visit in hourly blocks of time to help with non-medical tasks, including:
- Laundry assistance
- Light housekeeping
- Grocery shopping
- Meal preparation
- Escorting on errands
- Assistance with pets and plants
Our caregivers are each screened by Meadowlark so you can feel completely safe and secure when they enter your home.
Appointments are available on a daily, weekly or monthly basis, and are billed at an hourly rate. To get started, a licensed nurse or social worker will visit with you to discuss your needs and set up a customized schedule.

Parkinson’s Program of Manhattan
This program is a comprehensive approach to living with Parkinson’s, with components including:
- Physical strength and mobility
- Swallowing function (assessment and treatment)
- Speech (evaluations and treatment)
- Medication monitoring
- Nutritional counseling
- Evaluation of motor and behavioral functions
- Resident/family education
- Counseling and support groups
- Exercise programs
- Group classes
- Resource library
- Support group
Meadowlark hosts a monthly support group for Parkinson’s patients and their caregivers. This is a free service, open to the public, and offered on the first Thursday of each month from 2 p.m. to 3 p.m. in the Meadowlark Hills Community Room .
In 2007, Meadowlark received an Excellence in Parkinson Care Accreditation and became a Parkinson Proficient Community, as recognized by the National Parkinson Foundation. For Meadowlark’s specially trained nurses, social workers, aides and therapists, the goal is to help patients manage their Parkinson’s while maintaining maximum independence and quality of life.
This program is available to residents, as well as individuals in the Manhattan community.

Palliative and Hospice Care
Working specifically with patients who have chronic conditions (palliative patients) or terminal conditions (hospice patients), Meadowlark’s Palliative Care Team knows how to help relieve pain, increase comfort and restore quality of life. The Team includes Meadowlark’s on-campus physician, a registered nurse, a social worker, a chaplain, and your primary care physician — who all work together using a multidisciplinary, resident- and family-centered approach.
Palliative and hospice care are often appropriate for people with:
- Heart, lung and neurological diseases
- Alzheimer’s or dementia
- Stroke
- Coma
- Liver or kidney failure
- Cancer
If you ever need this type of specialized care, the Team will work with you and your physician to offer clinical expertise, education and emotional support.

Flint Hills Low Vision Services
One in six adults over age 45 experiences problems with low vision. If visual impairment is altering your daily activities, Flint Hills Low Vision Services can help. By identifying issues caused by vision loss, our occupational therapists can help build an individualized plan for you to live safely while retaining the highest degree of independence possible.
Services include a comprehensive assessment, instruction on how to maximize vision for day-to-day living, and recommendations for adaptive equipment. Services are available to both residents and non-residents.

Counseling Services
Dietary Consultations
Work with a registered dietitian on nutrition issues including diabetes, weight loss, and lowering fat or cholesterol.
Emotional Support
Everyone can use someone to talk to when life becomes stressful. Our clinical social workers offer confidential, compassionate help with life transitions, depression, anxiety and grief. Both private and group counseling are available for residents, family members, caregivers and Manhattan community members. These services usually are covered by Medicare. Supplemental insurance is accepted.
Support Groups
In difficult, emotional times, having the support of people in a similar situation can be invaluable. Our support groups focus on common issues, such as support for caregivers and coping with grief.
