Social Services Leader (Licensed Social Worker)

Meadowlark Hills is seeking a Licensed Social Service Leader.

The Social Services Leader (SSL) ensures the psychosocial, emotional, spiritual and other social needs of the residents are met.  Additionally, the SSL shares the responsibility of promoting and sustaining an environment that humanizes and individualizes each resident. 

Minimum 5 years similar experience and/or education required. The Social Services Leader is expected to have acquired the necessary knowledge and skills to perform the job satisfactorily in accordance with current standards, guidelines, and regulations of Meadowlark Hills.

A license of master's social work in the state of Kansas is required.

This position exercises supervision of Social Service functions performed by Social Service Designees. The Social Services Leader will predominantly maintain daytime business hours; however, some nights and weekends will be required.

ESSENTIAL JOB FUNCTIONS

  • Motivate residents to make healthy adjustments to the area in which they live by their participation in activities and social events.
  • Investigate, uncover and mitigate any problems which might interfere with the residents’ socialization and participation in activities.
  • Be available to help residents who feel a need to share personal problems, and make recommendations for supporting services or resources.
  • Have daily contact with residents, providing residents with opportunity to make vocal their problems and re-assurance that staff is interested in their well-being.
  • Find solutions to issues concerning residents, families and the organization.
  • Report all grievances to Administration.
  • Help any resident which displays psychosocial adjustment difficulty after moving to long term care.
  • Maintain current and accurate evaluation documentation for each resident.
  • Comply with all state and federal requirements pertaining to complete and accurate documentation.
  • Assists residents or responsible parties in processing forms or applications in the effort to obtain outside services. 
  • Evaluate residents’ home environment.
  • Assists and participates in support groups, committees and other meetings as designated by Administration.
  • Act as a resource to family members and a liaison between them and the organization.
  • Schedule appointments as needed with residents, families or staff.
  • Works with Household Coordinators to help identify medically-related social service needs of the resident and ensure that the needs are met by the appropriate disciplines.
  • Complete Life Style Interviews

  • Supports team in meeting all job requirements, provides timely feedback, and actively participates in being a learning and teaching organization. Willingness to continue to learn and seek improvement.
  • Attends in-service training programs as established by Administration.
  • Responsible for staying abreast of current laws, regulations, policies, procedures and best practices and adapting to company and industry changes.
  • Responsible for performing other duties as assigned.
  • The SSL may assist with move-in processes as needed.  This may include tours, paperwork, and physical moves of new or existing residents.

Education:  Minimum of license of bachelor’s social work in the state of Kansas is required. 

Personal Skills: The skills necessary for successful performance of the SSL include, and not limited to:

  • Must have the knowledge and the ability to apply professional principles, procedures, and techniques.
  • Must have thorough knowledge of available resources, tools and services available to residents and families.
  • Must demonstrate effective verbal and written communication skills along with proper telephone etiquette.
  • Performs in a tactful and professional manner with a strong focus on Person-centered care.

Supervision: This position exercises Social Services supervision of Social Service functions performed by Social Service Designees.  This position reports directly to the Health Services Director.

Financial Accountability:  Responsible for making optimal use of all resources including time, while respecting residents’ and Meadowlark Hills’ property.

Working Environment: The Social Services Leader works primarily in a climate controlled office setting with controlled lighting, temperature and air quality. The position operates standard office equipment including, but not limited to computers, fax machines, copiers, printers and telephones. At times this position is required to partake in services or activities outdoors and/or off campus.

Physical Demands:  This position requires good physical and mental health.  The SSL must possess manual dexterity for typing and operating a computer.  The SSD may be required to lift, sit, stand, bend or move intermittently during working hours.

Work Hours: The SSL will predominantly maintain daytime business hours, however, some nights and weekends will be required.  The SSL or his/her designee is expected to be available to the Social Services Team at all times.
