Household Nurse, LPN

Meadowlark Hills is currently seeking LPNs to work in our Health Care households. Full-time positions are available.

 

POSITION SUMMARY

 The Household Nurse performs routine daily nursing functions and is responsible for providing primary care to each resident, including assessment, treatment, care planning, passing medications, monitoring direct care team members, and medical care evaluations. The Household Nurse works closely with the clinical coordinator and the household team to ensure regulatory compliance, while focusing on person-centered care. The Household Nurse provides supervision, leadership, and guides the household team while promoting individual and team learning in the household. The Household Nurse maintains a professional work relationship with the household team and residents, and remains knowledgeable of residents’ responsibilities and rights, including the right to determine their own preferences.

ESSENTIAL JOB FUNCTIONS

  • Promotes residents’ well-being, by providing each resident with nursing care or emergent care services; assesses needs, establishes and implements the resident’s plan of care, and advocates for resident and family preferences.
  • Implements and ensures clinical policies, procedures, and operations in the household are in compliance with federal, state, and local regulations, the State’s Nurse Practice Act, and professional standards of nursing care while honoring person-centered care.
  • Maintains working relationships with physicians and other medical personnel through clear and effective communication.
  • Observes the status of residents as necessary, and ensures proper documentation, medication, and treatment administration is provided for quality care. Communicates and coordinates with the clinical coordinator, household team, physician, and other Meadowlark Hills team members to assure safety and continuity of care.
  • Recognizes early signs of changing conditions, uses all available and appropriate resources, and seeks consultation with household clinical coordinator and/or director of nursing services when needed.
  • Maintains a safe, comfortable, and functional home environment by assessing the household for potential hazards and encourages continuous quality improvement.
  • Implements practices, utilizes appropriate techniques regarding sanitation, infection control, fire safety, disaster preparedness and emergency care.
  • Supports representation of the household team on all appropriate councils and teams, investigates and reports all incidents and accidents, observes the team for safe work habits, and addresses needs and concerns.
  • Provides leadership and vision to the household by serving as a role model for core competencies and professionalism.
  • Supports a learning environment, sharing resources, knowledge, expertise, opportunities and achievements with the household team. Advocates for needs of the household and team as needed.
  • Participates in the performance evaluation process providing feedback through coaching and counseling, celebrating individual and team success.
  • Performs leadership and mentoring responsibilities including but not limited to: plans, assigns, directs, teaches, and trains household team. Addresses complaints and resolves concerns, and participates in team problem solving methods, mediating conflicts concerning the nursing team, residents, and family members as needed.

  • Attends in-service training programs.
  • Responsible for staying abreast of current laws, regulations, policies, procedures and best practices and adapting to company and industry changes.

POSITION REQUIREMENTS

Experience:  Minimum two (2) years of similar experience and/or education required. A Household Nurse is expected to have acquired the necessary knowledge and skills to perform the job satisfactorily in accordance with current standards, guidelines, and regulations of Meadowlark Hills.

Education:  High School Diploma or GED equivalent required. Licensed Practical Nurse or Registered Nurse acknowledged by the state of Kansas required.

Licensing Requirements:  Licensed in the state of Kansas as a Licensed Practical Nurse (LPN) or a Registered Nurse (RN) required.

Personal Skills: Accurate, detailed, and dependable, with a commitment to the highest of clinical outcomes. Strong knowledge and observational skills with regards to physical and medical conditions, coupled with the ability to determine effective responses and treatment. The Household Nurse must have strong leadership, initiative, teaching, coaching, facilitating, communication, and team building skills. Flexibility and patience are needed to multi-task, prioritize, and adjust to changing situations and medical needs. Requires the ability to provide clear directions and expectations, while offering effective feedback.

Problem Solving: Critical thinking skills are needed in problem solving issues involving, but not limited to: resident care, interpersonal relationships, conflict resolution, and facilitation with household team, residents, family, and guests.

Decision Making: Logical sound judgment needed when making decisions and taking appropriate action concerning resident care. Maintains compliance and safety standards while focusing on a person-first philosophy.

Supervision:  This position directly leads CNAs and CMAs, and may indirectly lead other household team members when required.

Financial Accountability:  Responsible for making optimal use of all resources including time, while respecting residents and Meadowlark Hills’ property.

Personal Relations: Honors all resident and family preferences through support, friendship, kindness, sincerity, communication and relationship building. The Household Nurse works closely with all members of the household team, leadership team, household coordinator team, clinical coordinators, operations leaders, life enhancement, social services, human resources, residents, families and guests.

Working Environment: The Household Nurse may be in close contact with individuals who have infectious diseases. Additionally, the Household Nurse may be subject to the handling of harmful chemicals, infectious waste, unhygienic conditions, and potentially hazardous compounds, solutions, supplies, equipment and electrical shock. The Household Nurse must observe rigid, standardized guidelines to guard against disease, accidents, and other dangers, wearing protective equipment when needed. The Household Nurse is often subject to stressful, uncomfortable, and emotional situations while helping residents and families. A Household Nurse primarily works in a climate control setting.

Physical Demands: The Household Nurse position requires physical activity and continuous movement throughout the workday in order to perform daily duties, spending long periods of time standing, walking, and assisting residents. Requires the ability to stand, walk, push, pull, carry, move, bend, kneel, squat, reach, stretch, and lift a minimum of 50 lbs. The Household Nurse must use universal precautions in all situations. This position requires the ability to pull up or reposition residents in bed, lift supplies, place equipment on supports, and bend over to perform certain duties. The Household Nurse must also be able to quickly maneuver throughout halls, stairways, and resident/guest rooms in response to emergencies. In the event there is a need to evacuate the building, heavy lifting may be required to carry residents to safety. Must be able to operate office equipment (phone, computer, printer, copier, fax machine, etc), and various equipment and mobility devices (wheelchair, walker, gait belt, mechanical lift, etc) used by/for residents. These operations must be performed with normal visual range, acuity, color determination, and the ability to distinguish letters and numbers. Good oral communication skills, the ability to speak clearly, and the ability to listen attentively with appropriate hearing acuity is required.

Work Hours:  Residents and guests require 24-hour a day care; so the ability to work nights, weekends and holidays is required. Primary work hours are dependent upon the shift(s) needed for full-time and part-time positions. Prefer no more than 40 hours per week. The Household Nurse is on an on-call rotation and subject to emergency call outs on short notice. Hours may vary depending upon residents’ needs.  Adjustments to the schedule are made to accommodate these demands when necessary.

 

IV certification or willing to obtain certification is preferred.
