Health Care Household Options

The 3 Steps to Move

1. Contact Social Services and complete an application for residency.

2. At this time, it is required that you schedule your Health & Lifestyle Review (HLR). This is a personal and private assessment of your current health to ensure appropriate care as necessary in the future.

3. Provide a letter from your primary care physician recommending/advising the necessary levels of care.

Levels of Service

Level One
Performs bathing, dressing and medication administration with minimal support. Shopping assistance is also provided.

Level Two
Receive assistance with care in order to meet daily needs including one or both of the following: dressing or personal hygiene.

Level Three
Resident requires on-going total care with two or more of the following activities: bathing, toileting, continence program, dining assistance or transfers.

Level Four
In addition to those noted in Level Three, resident receives specialized and/or individualized care including catheter or ostomy care, IV treatment, behavioral management, feeding, oxygen or other individualized therapy.

Level Five
Resident receives Medicare-certified care as designated by a Medicare physician and in keeping with Medicare regulations.

Help ensure that all residents receive the care they need. Donations to the Good Samaritan Fund help assist residents needing health care that have depleted their available resources.
